Probiotic CFU Count: Does Higher Really Mean Better?

Probiotic labels love to compete on one number: CFU count. But NIH’s own guidance says that number isn’t what determines whether a probiotic actually works — the specific strain is. Here’s what actually matters when you’re comparing two bottles on a shelf.

Why NIH Says the CFU Number Isn’t the Most Important Thing

NIH’s National Center for Complementary and Integrative Health puts it plainly: different types of probiotics may have different effects, and if a specific kind of Lactobacillus helps prevent an illness, that doesn’t mean another kind of Lactobacillus — or any Bifidobacterium probiotic — will do the same thing. In other words, probiotic effects are tied to the exact strain, not the broader family of bacteria it belongs to, and definitely not to how many billion of them are packed into a capsule.

That specificity cuts both ways. NIH also acknowledges that in most instances, we still don’t know which probiotics are helpful and which are not, how much people would need to take, or who’s most likely to benefit. That’s a fairly candid admission from the agency that funds this research — and it means a big CFU number on a label isn’t backed by the same kind of certainty the marketing implies.

So if the number on the label isn’t the deciding factor, what is?

What Actually Has Evidence Behind It: Specific Strains, Not Big Numbers

Strain-level evidence does exist for some things. A 2017 review of 17 studies with 3,631 participants found that taking probiotics alongside antibiotics was associated with roughly half the risk of antibiotic-associated diarrhea, though NIH describes that conclusion as tentative because the underlying studies were only moderate quality. Even within that finding, the benefit hasn’t been shown consistently across every strain or every age group — a review focused on older adults didn’t find the same effect, and NIH notes it’s unclear whether that’s because the probiotics genuinely don’t work as well in that group or because there simply aren’t enough studies yet.

Jarrow’s Jarro-Dophilus EPS lists eight strains and 55 billion CFU per capsule, including a Lactobacillus rhamnosus and Lactobacillus helveticus pairing that, according to the manufacturer’s own research arm, has been used in a couple dozen clinical studies over the years. That’s worth noting, but it’s not the same as saying all eight strains in the capsule carry that same weight — this research didn’t turn up independent clinical studies for each of the other six strains individually, and a product listing eight strains doesn’t mean all eight have been tested together or separately to the same degree.

There’s a second marketing claim worth checking too: the enteric coating.

Does Enteric Coating Actually Protect Probiotics From Stomach Acid?

There’s real research behind this one, at least in part. In one published study on freeze-dried, enteric-coated Lactobacillus acidophilus capsules, the coating held up without breaking down in simulated stomach acid while dissolving properly once it reached simulated intestinal fluid, and the coating process itself recovered about 95% of the live bacteria as viable cells. Uncoated, plain powder probiotics tested the same way often show far weaker survival once exposed to stomach acid.

But that doesn’t make enteric coating a requirement for a probiotic to work. Some of the most heavily studied probiotic strains in the world have demonstrated real effects in clinical trials without any special acid-resistant coating — their own natural tolerance to stomach acid was apparently enough. So enteric coating looks like a reasonable engineering choice that can help under lab conditions, not a make-or-break feature that separates effective probiotics from useless ones.

None of this means CFU count is meaningless, though.

So Is a Bigger CFU Number Ever Worth Paying Attention To?

Not entirely meaningless — some bacteria are lost on the way through the digestive system, so a probiotic does need enough live organisms to begin with for any of them to survive the trip. But that’s a floor, not a ranking system. Once a product clears that basic threshold, a bigger number doesn’t tell you whether the strains inside have actually been tested for the thing you’re trying to address.

A more useful habit than comparing CFU counts is looking up the specific strain names on the label — the letter-and-number codes like the ones on Jarrow’s Jarro-Dophilus EPS — and checking whether that particular strain has been studied for your specific concern. The enteric coating on a product like this is a reasonable design choice, not proof that it outperforms a supplement without one.

The Bottom Line

A bigger CFU number on a probiotic label feels like a straightforward way to compare products, but NIH’s own guidance doesn’t support using it that way — the evidence that exists is tied to specific strains, not totals. Some strains, like the rhamnosus-helveticus pairing found in several products on the market, have a real body of research behind them for things like antibiotic-associated diarrhea. Others carry a CFU count and a marketing claim without the same paper trail. Enteric coating can genuinely help bacteria survive stomach acid, but it isn’t the deciding factor either. The number that matters least is usually the biggest one on the front of the label.

Next time you’re standing in that aisle, skip the CFU count and look up the strain names instead.
